NEW ENROLLMENT: There will be a one-time enrollment fee of fifty dollars. This fee is non-refundable.
SUPPLY FEE:
A yearly fee will be collected in January, of twenty-five dollars. (This is per enrolled child.) If you start your child anytime during the year, you will still have to pay the supply fee when enrolling and again in August. This fee is non-refundable.
TRIAL PERIOD:
There is a two-week trial period at the beginning of childcare, to make sure that the child and the childcare do not have any conflicts. At anytime during the two-week trial, either the parents/guardians or the provider may terminate care with one days notice. After the trial period of two-weeks is over, removing the child applies as in the contract and policies.
HOURS:
The hours of operation for childcare are Monday through Friday 7:00 am to 5:00 pm, during the school year.
My child will be attending ___________ each week. My child will be dropped off at ____ am and picked up by _____ pm. If they are dropped off later or picked up earlier than the times stated, the rate is the same. If they are dropped off earlier or picked up later than the scheduled times, a fee will be charged.
PAYMENT OF TUTION:
The rate is $160.00_per week for full-time care. Payment is due in full whether or not your child is in attendance. (Parent's/Guardian's will provide diapers, wipes, baby food, formula & a large diaper geni refill as needed.) Payment must be in cash and paid on Fridays before 5pm, in advance to childcare services being provided for the next week. Every August the tuition may increase five dollars; this is a cost of living increase. All rates are subject to change with a written two-week notice.
LATE PAYMENT OF TUTION:
A charge of $10.00 per day, per child will be collected for payment not posted on Friday by 5pm, for the following week.
EXTRA DAY CHARGES: $30.00 per day, per child
EXTRA HOUR CHARGES: $5.00 per hour, per child.
LATE PICK UP CHARGE:
Late charges will be collected the same day as services rendered. The late fee charge is $1.00 Per every minute late, this must be paid in cash the same day. If you notify me in advance that you will need additional time, either in the morning or evening. The extra hour charge will apply of $5.00 per every additional hour, per child.
SICK DAYS/ PERSONAL DAYS FOR CHILDCARE PROVIDER:
The Daycare Provider is allotted five sick days/ personal days, with pay, per year. Parent's/ Guardian's will be notified of Providers sick/personal day no later than 6:30 am that day.
PROFESSIONAL DEVELOPMENT DAYS:
When there is a conference or meeting during the week that the provider has to attend, Parents/Guardians will be notified in advance. Parent's/Guardian's will be responsible for alternate care on these days. These days will be paid days for the Provider.
BAD WEATHER DAYS:
When the Mansfield Independent School District closes school due to weather, Childcare will also be closed. The Provider will inform Parent's/Guardian's if the childcare will be open for the day, open half the day or closed all day. Due to power outages or severe weather, where the provider is unable to keep a comfortable & controlled environment, childcare will be closed. These days will be paid days for the Provider.
BEREAVEMENT:
The provider is allowed one week paid bereavement for immediate family members. The provider is allowed two days paid bereavement for any other family members or friends. Parents/Guardians will be responsible for finding alternate care.
JURY DUTY:
If the provider had to attend jury duty selection, these will be paid days. If the provider is selected to sit on a jury, these will be paid days for the provider for the length of the trial. Parents/Guardians will be responsible for finding alternate care.
HOLIDAYS CHILDCARE IS CLOSED: full payment is due for weeks that include holidays. (If holiday falls on Saturday=childcare May be closed Friday in observance of holiday) (If holiday falls on Sunday= childcare May be closed Monday in observance of holiday)

TEACHERS & SUMMER CHILDCARE:
Parent's/Guardian's have the option to keep the child enrolled in the childcare during the summer and to pay half the weekly tuition, (per child) as per the contract/policy. This half price tuition is non-refundable for any reason. This half price summer tuition does not go towards the regular tuition. This option will keep a secured spot for the child when fulltime care/school resumes. If the Parent’s/Guardians do not wish to pay during the summer, they can remove the child from care, by submitting their termination notice in writing. However the child’s spot will be filled and will not be open when school restarts.
REMOVING CHILD FROM CHILDCARE:
The Parent's/Guardian’s: May terminate this contract by giving a two-week written notice. Tuition for the two week written notice of termination is due, even if your child does not continue to attend during that time, when written notice is given. When removing child from childcare, all tuition must be current, including the payment of the two-week notice tuition. If parent’s fail to pay two week notice tuition, they will be responsible for all legal fees, court fees, reimbursement for provider’s time off work and any other cost pertaining to the collection of the two week tuition.
The Provider: May terminate this contract by giving a two-week written notice. Tuition for the two week written notice of termination is due, even if your child does not continue to attend during that time, when the written notice is given to the parent’s/guardians. When removing child from childcare, all tuition must be current, including the payment of the two-week notice tuition. If parent’s fail to pay two week notice tuition, they will be responsible for all legal fees, court fees, reimbursement for provider’s time off work and any other cost pertaining to the collection of the two week tuition
The Provider: May immediately terminate this contract without giving notice, if the Parent's/Guardian's do not make payments when due, do not pick up an ill child when notified to do so, or the child is a risk to the other children in care, disrupts the daily running of the childcare center, or the provider sees any other need to do so. ______ Mother initial ___________Father initial
The signature of the Parent's/Guardian's to this contract indicates that they have read and agree to abide by the written contract and policies of the Provider. The signing of this contract indicates that they understand that they are responsible for any and all court fees, attorney fees, legal fees, missed day of pay fees for the provider to attend court or any other cost incurred while enforcing this contract. That the Provider may change the contract/policies from time to time, by notifying the Parent's/Guardian's in writing of the changes that apply to them. _________________________ ____________________________
