CHILD CARE WILL BEGIN ___________________.   Drop off at ______am.   Pickup by____ pm. The weekly tuition is $160.00 for full time.  This tuition is due on Fridays by 5pm, for the following week. A late fee of $10.00 per day will be collected if not paid by 5pm on Friday. The fee for early drop off is $5.00 per hour, per child.  The fee for late pickup is $1.00 per every minute, per child, or $5.00 an hour with advance notice given. The fee for extra days is $30.00 per day per child. All rates are subject to change with a two-week written notice.

MEALS / SNACKS: (for children 1yr and older)

The provider will furnish a morning snack (9:00am), a lunch (11:00am) and an afternoon snack (3:30pm)

 

PEANUT OR PEANUT PRODUCTS:

Peanuts and/or peanut products are used at this childcare.  Allergies to this product in young children may not be known yet.  Allergies to this product may present itself while at childcare.  If an allergic reaction does happen, all emergency procedures will be taken to assist your child.  If you already know your child has an allergy to this product (or any other allergy) you need to inform the childcare immediately.

DAYS PAY IS EXPECTED FOR:

Tuition is due for Provider sick days, Bereavement Professional days; bad weather days and all holidays listed in the contract.          

CHILD ILLNESS:

Children must be kept home if he/she has a fever of 101 degrees or higher. If he/she shows signs of a communicable illness. If he/she is unable to interact with other children. If he/she is unable to participate in the childcare program, due to not feeling well. Green drainage is considered contagious in most cases and the child may not return until drainage is clear. If child gets ill while in care, parent's/guardian's will be notified and must pick up child within one hour.  Child may not return until fever free for 24 hours without the aide of fever reducing medicines. If child is pre-medicated to hide fever or illness upon arrival, this is grounds for immediate termination of childcare.

MEDICAL CONDITIONS:

ABSENT / LATE:

When your child will be absent or late, it is required that you inform the provider at least one hour prior to scheduled arrival time.  (This helps in planning food and activities.)

SICK DAYS/ PERSONAL DAYS FOR PROVIDER:

The Provider is allotted five sick days/ personal days per year, with pay.

PROFESSIONAL DEVELOPMENT DAYS:

It may be necessary for the Provider to attend training on weekdays/work days. These are paid days for the Provider.  Parents/Guardians are responsible for alternate care on these days. 

BAD WEATHER DAYS:

When the Mansfield Independent School District closes school due to weather, Childcare will also be closed. The Provider will inform Parent's/Guardian's if the Childcare will be open for the day, open half a day, or closed all day. Due to power outages or severe weather, where the provider is unable to keep a comfortable & controlled environment, childcare will be closed.  These days will be paid for the Provider.

BEREAVEMENT:

The provider is allowed one week of paid bereavement for immediate family members.  The provider is allowed two days paid bereavement for any other family members or friends. Parents/Guardians will be responsible for finding alternate care.

 

JURY DUTY:

If the provider has to attend jury duty selection, these will be paid days.  If the provider is chosen to sit on a jury, these will be paid days for the provider for the length of the trial.  The parents/Guardians will be responsible for finding alternate care.

 

TEACHERS & SUMMER CHILDCARE:

Parent's/Guardian's have the option to keep the child enrolled in the childcare during the summer and to pay half price weekly tuition, per the contract/policy. This option will keep a secured spot for the child when fulltime care/school resumes. This deposit is non-refundable if you change your mind and decide not to return to childcare.  If the Parent’s/Guardians do not wish to pay the holding fee for summer care, they are required to give a minimum of two weeks notice to remove the child from care.  The child’s spot will be filled and will not be open when school restarts.

SUBSTITUTE CARE:

Parent's/Guardian's are responsible for substitute care when the Provider is unavailable.

PETS:

If any pet is acquired, Parents/Guardians will be notified and advised how interaction with the children and the pet will be handled.  Currently there are animals residing at the childcare.  There is documentation of current vaccinations and a statement of health on each pet located at the childcare.

DISCIPLINE:

Children learn to feel secure about themselves and others in a secure and manageable environment. The children will receive positive guidance, redirection and distraction for undesirable behavior.  When necessary, the child will be placed in time out, as a last resort to undesirable behavior.

 

POTTY TRAINING:

The parent's/guardian's and the provider must agree that the time is right for the child to begin.  Once potty training is started, wearing of elastic waisted pants only will be allowed.  Parent's/Guardian's must provide several pairs of thick training pants and two complete changes of clothing. Parents must sign a Potty Training contract.

LAYING POSITION:

Infants not yet able to turn over on their own must be placed in a face-up sleeping position, unless the child’s parent presents written documentation from a health-care professional stating that a different sleeping position is allowed or will not harm the infant.

PERSONAL ITEMS FROM HOME:

Your child will need one full change of clothes to be left at childcare.  This outfit needs to be weather appropriate. Any other items (toys, games, food, books...ect) need to stay at home, unless previously arranged with the Provider.

IMMUNIZATIONS:   (Copy of current shot record)

Immunizations are a state requirement.  If they do not have current immunizations, it is a cause for immediate termination or exclusion from care.  If they are not current they must have a doctor's note or a written religious objection. If parent/guardian’s refuse to have their child receive shots, for any reason, they must obtain a form from the Department of Health and have it on file with the provider.

 

MEDICATIONS:

State law prohibits the provider from administering medication without a completed and signed medication form from the parent/guardian.  Medicines must be in the original container, with the child's name on the container.  If the child is taking any medications, childcare provider must be made aware of the reason for the medication, type, symptoms and exact dosage to be given and last dose given. Any child under the age of two must have a doctor’s note to take any pain or fever reducing medication.  This note must state type of medication to administer and dosage amount for that child. (Any diaper or teething medication must also be listed on the medication form stating, “Apply as needed”.)

 

HEAD LICE:

If a child has head lice, the entire family needs to be treated, even if the rest of the family does not show signs. Child must be nit/egg free (70) seventy hours (or) five days before returning to childcare.  (I recommend the following to be done.)  The child’s hair needs to be slathered in “Mayonnaise” and left to sit for at least four hours.  After four hours, the hair needs to be washed with “DAWN” dishwashing liquid.  You will need to remove all the nits and eggs still left on hair. This process needs to be repeated 10-14 days after initial treatment.  All of the child’s clothes and bedding should be washed in hot water.  After being washed, clothes need to be run through two cycles in the dryer.  Any of the child’s items not able to be washed, need to be placed inside plastic and sealed for two weeks. 

 

BREATHING TREATMENTS: 

Provider is not certified to administer breathing treatments.  If the child requires breathing treatments, the child must remain at home or the parent’s must arrange alternate care.

ACCIDENTS/INCIDENTS:

Parent's/Guardian's will receive a report of any accidents/incidents that occur while child is in the care of the provider. Parents must sign and date the report. The report will be kept in the child's file and parents will receive a copy.

MOTOR VEHICLE TRAVELING:

Parent's/ Guardian's understand that in an emergency, the provider or emergency personal must Transport the children. By signing the policy form, they are giving their consent for emergency Transportation from the childcare.  (For field trips a separate form with details will be given.) When the child is in a motor vehicle, safety restraints must be worn.  The vehicle must have minimal insurance coverage required by the state and must be current.  As long as these precautions are taken, this relinquishes the childcare provider from any liability in case of a motor vehicle accident.

MEDICAL RELEASE:

For emergencies, a medical release must be filled out and notarized.  A statement of health must also accompany it, with an original doctor’s signature.

EMERGENCY PROCEDURES:

There are regular fire and tornado drills monthly. In the case of a tornado, we will be located in the closet. A safety plan is posted on the playroom wall, next to the door.   If an emergency should happen to either the child or the provider, 911 will be called.  If either the child or the provider has to be transported to the hospital, backup care will be provided until the parent's arrive.  Parent's/Guardian’s will be contacted by the back up provider and need to immediately pickup their own children from childcare. In the event we need to relocate due to a non-weather disaster, (fire, toxic spill, etc) I will post the address and directions on the outside door to where we will be relocated.  I will also try to contact you once the relocation process has been completed to inform you by phone, if possible, of the relocation spot.

TELEPHONE/ANSWERING MACHINE:

If the phone is not answered when you call and the answering machine picks up, there is nothing wrong.  The provider is just too busy to answer.  The ringer on the phone is turned off between 12pm-3pm due to quiet naptime period.  The Provider will return your call as soon as possible.

 

PERSONAL VISITS FROM THE PARENT'S/GUARDIAN'S:

Parent's/Guardian's are allowed to drop in unannounced.  The Parent's/Guardian's personal visit can be no longer than thirty minutes.  If Parent's/Guardian's wish to stay longer than thirty minutes, a criminal background check must be completed and a negative TB test on file with the childcare ahead of time.

POOL RELEASE:

A pool release must be filled out by parent's/guardian’s before any swimming activities.

PLAYGROUND EQUIPMENT & TOYS:

Parent's/Guardian's will be shown all toys and playground equipment in use at the childcare.  Parent's/Guardian’s will be informed about new toys or equipment used in the childcare.

 

PHOTOGRAPHS, WEBSITE ADVERTISING & VIDEO TAPING:

The Parent's/Guardian's understand that the child will be photographed or video taped occasionally by the provider or employees of the childcare. That the photo's or videos are sometimes used for training, website advertising and community purposes, without compensation. The child will not be identified by full name in any public showing.

DEVICES NOT ALLOWED IN CHILDCARE CENTER:

Cell Phones, Cameras, video cameras, video recorders, tape recorders, electronic communication devices equipped with cameras and recording features regardless of whether the instrument is self contained or incorporated into one or more of these devices.

PARENT/GUARDIAN CONFRENCES:

In May, the Provider will hold an optional conference time with each Parent/Guardian, to discuss any changes to the contract/ policies, concerns & goals for the child.

 

AUTHORIZATION TO PICK UP CHILD:

Parent's/Guardian's must have written documentation of who they want to pick up the child. This person must show a valid ID to the provider to remove the child from the childcare.   Telephone calls will not be observed to relinquish the child to a person not previously listed.  In the event of a divorce, the Provider cannot deny either parent/guardian the right to pick up the child, unless a copy of the custody order is on file with the provider. The custody order must state Custodial Guardianship and visitation rights to both parents.  If an issue arises during childcare hours, the Provider will contact the police to handle the situation and have the child removed from care immediately!

Tuition for the two week written notice of termination is due, even if your child does not continue to attend during that time, when written notice is given. When removing child from childcare, all tuition must be current, including the payment of the two-week notice tuition. If parent’s fail to pay two week tuition, they will be responsible for all legal fees, court fees, reimbursement for provider’s time off work and any other cost pertaining to the collection of the two week tuition

 

The Provider: May terminate this contract by giving a two-week written notice. Tuition for the two week written notice of termination is due, even if your child does not continue to attend during that time, when the written notice is given to the parent’s/guardians. When removing child from childcare, all tuition must be current, including the payment of the two-week notice tuition. If parent’s fail to pay two week tuition, they will be responsible for all legal fees, court fees, reimbursement for provider’s time off work and any other cost pertaining to the collection of the two week tuition

 

The Provider: May immediately terminate this contract without giving notice, if the Parent's/Guardian's do not make payments when due, do not pick up an ill child when notified to do so, or the child is a risk to the other children in care, disrupts the daily running of the childcare center or the provider sees any other need to do so.

The signature of the Parent's/Guardian's to this contract indicates that they have read and agree to abide by the written contract and policies of the Provider. The signing of this contract indicates that they understand that they are responsible for any and all court fees, attorney fees, legal fees, missed day of pay fees for the provider to attend court or any other cost incurred while enforcing this contract. That the Provider may change the contract/policies from time to time, by notifying the Parent's/Guardian's in writing of the changes that apply to them.
